• General Medical History - meaning a list of medications - and they are to treat what?
• Within the last year, if any medications changed, which ones, and when?  We don't wish to be nosy, but without the above information, we cannot find you the best answers for your specific needs, as each person has their own specific needs when it comes to the topic of insurance.
